Filter by Criteria Function

Hi guys, would like to seek advice on how can I go about filtering my list view results based on my search criteria

For instance, if I wish to search by Subject and/or Job Location, how can I do so? My data base is Google Spreadsheet via Google Forms.

I have a two apps for that in the gallery. Search for "List Picker Based Filter" and "Custom Filter Bar" in the gallery you will find them.
